Current Students

Physiology students will find downloadable PDF documents and forms (or links to forms) that will be needed at certain points during your graduate careers. These forms/documents will be updated as necessary so please check back periodically. Please note that a copy of each form needs to be kept in your folder in the graduate student office.

General Procedures and Forms

  • Rotation Evaluation Form (doc)
    Please have a faculty member fill out the Rotation Evaluation Form and return to GPC (Graduate Program Coordinator) by the end of each rotation.

  • Thesis Advisor Selection Form (pdf)
    PhD students are required to choose an advisor by the end of summer semester of 1st Year. Letter with advisor's and student's signatures is required. MS students are required to choose an advisor by the end of spring semester of 1st Year. Letter with advisor's and student's signatures is required.

  • Thesis Committee Selection Form (pdf) (on Graduate College Site)
    PhD students are required to choose a thesis committee by the end of Spring semester of 2nd year. MS students are required to choose and meet with a thesis committee in the Fall semester of 2nd year. MD/PhD students are required to choose a thesis committee by the end of Spring semester of G1 (3rd year).

  • Intent to Graduate (on Graduate College Site)
    Student must fill out the intent to graduate forms and register for PHYB 595 for the semester of defense. Departmental approval is required - contact GPC.

  • Thesis Manual and Submission Instructions (on Graduate College Site)
    Student must follow the guidelines in the Thesis Manual to format the dissertation. Once it is approved by the department, the student must submit the thesis electronically to the GC (Graduate College) before their deadline for graduation in the semester one intends to graduate.

PhD Thesis Components

  • Preliminary or Oral Qualifying Exam
    Committee Recommendation Form (pdf) (on Graduate College Site)

    Student composes members of Prelim Committee and gets it approved by GEC (Graduate Education Committee) at least 4 weeks before anticipated exam, but one semester before is best. It is best to have the committee members approved by GEC before contacting them. Student fills out a Committee Recommendation Form (above) with anticipated exam date (does not have to be exact). Once student completes the form and gets the appropriate signatures, she/he gives the form to GPC, who then sends it to the Grad College (MC 192). Student needs to determine the date and reserve a room, and notify GPC of exact date in order for the file to be prepared for the Chair of the Committee. Student needs to notify GPC of any changes in date. Within 2 weeks from sending the Comm Rec Form, the Grad College will send an Exam Report that needs to be completed by the Committee. After the meeting, the Chair will give the file back to GPC, with a completed exam report, which goes back to the Grad College within 48 hrs of the exam. Student gets a copy. About 2 weeks after the exam report is sent to the Grad College, they will send a letter addressed to student that they are formally admitted to the Program, GPC places the original in student's mailbox, and keeps a copy for the file, and distributes a copy to Dr. Stocco and Dr. Kitajewski.

    Recommended schedule for prelim for PhD students: by March 31st of 2nd year. Recommended schedule for prelim for MD/PhD students: by March 31st of the first year of graduate work.

  • Pre-Thesis

    Student composes the Thesis Committee and gets it approved by GEC (Graduate Education Committee) at least 3 weeks before anticipated exam. It is best to have the committee members approved by GEC before contacting them. Student fills out a Committee Recommendation Form (above) with anticipated exam date (does not have to be exact). Once student completes the form and gets the appropriate signatures, she/he gives the form to GPC. Student needs to determine the date and reserve a room, and notify GPC of exact date in order for the file to be prepared for the Chair of the Committee. Student needs to notify GPC of any changes in date. This is an internal exam, no documents are sent to GC.

    Recommended schedule for prethesis for PhD students: end of spring semester of 2nd year. Recommended schedule for prethesis for MD/PhD students: end of spring semester of first the first year of graduate work (end of 3rd year).

  • Mid-Thesis Seminar
    Student must register for PHYB 595 the semester of their mid-thesis seminar. Departmental approval is required - contact GPC. Student is responsible for finding their own room and setting up a time that works for all committee members. Once the date, time and location are established student must notify GPC of the date. At least 3 weeks before the seminar, student must send title and abstract to GPC in order for GPC to create and distribute a flyer. GPC prepares the student file for the Chair of the Committee. Student must notify GPC of any changes in date of the seminar. Thesis Committee meeting takes place immediately following the Seminar.

    Recommended schedule for mid-thesis: Feb. 28 of the fourth year.

  • Thesis Defense
    Committee Recommendation Form (pdf) (on Graduate College Site)

    At least 4 weeks before anticipated defense (sooner is preferred) student fills out a Committee Recommendation Form (above) with anticipated exam date (does not have to be exact). GPC sends it to GC in order to receive exam report and certificates of approval on time. At least 4 weeks before the defense, student must send title and abstract to GPC in order for GPC to create and distribute a flyer and post info on UIC events. After defense, thesis corrections and thesis submission to GC take place. Theses are submitted electronically, and must be submitted to GC before their deadline for graduation this semester. Classes

  • Late Registration
    Student fills out a GRAD COLLEGE Registration Revision Form, gets signature of DGS (Director of Graduate Studies), gives it back to GPC, who then send it to the GC.

  • Missing Grade - supplemental grade report required from instructor

  • Petitions for Class Drop/Add
    Please contact GPC for the appropriate GC forms

  • Required Seminar Attendance
    Students are required to register for PHYB 591 and to attend at least two-thirds of the Departmental Seminars, usually scheduled for 10:00 am on Fridays, Fall and Spring Semesters during their graduate career. Attendance sheets are taken to the seminar each week by the Graduate Student Representative and submitted to GPC after each seminar.
